Surrender Request
These forms are to gather information to see if we have a foster home that can take your dog or cat. This is not a guarantee that we will take your pet into our rescue. Please fill out the information truthfully so that we can find the best fit for them if we are able to take them in!
For dog surrenders 4 months and older, an evaluation will be scheduled prior to the dog being listed for a foster. Evaluations are done on Saturday mornings in Tarentum, PA. Someone will reach out to you to discuss the surrender application and schedule you to bring the dog for an evaluation. *Vetting paperwork and proof of rabies must be brought to the evaluation.
